We are seeking a skilled Occupational Therapist to join our team in providing high-quality, patient-centered therapy services in the comfort of patients' homes. As an essential member of our interdisciplinary team, you will assess patients' daily living skills and develop tailored therapeutic programs to improve their functionality, promote community reintegration, and enhance overall well-being.
Responsibilities
- Assess patients' daily living skills and develop individualized treatment plans to address functional deficits
- Conduct thorough evaluations to identify areas for improvement and create effective interventions
- Collaborate with healthcare teams to ensure seamless coordination of care and maximize patient outcomes
- Provide direct patient care, including education on proper techniques and adaptive strategies for daily activities
- Document patient progress and report findings to healthcare teams as needed
- Participate in quality improvement initiatives to advance best practices and optimize patient satisfaction
Requirements
- Master's degree in Occupational Therapy from an accredited program
- Current licensure as an Occupational Therapist (OT) in good standing
- Minimum 6 months of experience in home health or similar setting
- Excellent communication and interpersonal skills, with ability to work effectively with diverse populations
- Strong analytical and problem-solving abilities, with capacity to adapt to changing situations
- Valid driver's license and reliable transportation for travel to patients' homes
