Become a key member of our community-focused healthcare team and prioritize patient well-being. As a Home Health Speech Language Pathologist, your responsibilities will include:
- Evaluating, directing, and providing speech/language pathology services to patients in their homes or facilities.
- Collaborating in the development and periodic review of treatment plans and care plans.
- Utilizing professional expertise and judgment to assess and treat disorders affecting speech, voice, language, hearing, and swallowing, with the goal of preventing, identifying, evaluating, and minimizing their effects.
Key Responsibilities
You will administer and interpret diagnostic tests and therapeutic treatments, including audio logic screening. Additionally, you will provide instruction and training to patients in using alternative communication systems when necessary. Your role also includes offering counsel and instruction to patients, their families, and healthcare staff, as well as maintaining and submitting required documentation.
Care Coordination and Compliance
You will participate in care coordination activities and discharge planning. It is essential to maintain the highest standards of professional conduct regarding confidential information. You are also expected to attend, participate in, and/or conduct internal staff development programs, pursue continuing education as required by company policy, and meet certification requirements for clinical competence in speech-language pathology or audiology granted by ASHA.
Required qualifications include meeting the education and experience requirements for Certification of Clinical Competence in Speech-Language Pathology or Audiology granted by ASHA, a minimum of six months' experience as a speech therapist/speech-language pathologist, home health experience preferred, current and unrestricted license, current CPR certification, and good organizational and communication skills. A valid driver's license, auto insurance, and reliable transportation are also required.
