Speech Language Pathologist, Home Health Per Diem at CenterWell Home Health
As a Speech Language Pathologist in Home Health, you will provide evaluation, direct care and therapeutic services to patients in their homes or facilities. You will work with healthcare teams to develop treatment plans and offer guidance and training to patients and their families to address speech and language disorders. This role requires strong organizational and communication skills, ensuring patient confidentiality and care standards are maintained.

As a Speech Language Pathologist in Home Health, you will:
- Evaluate, direct and provide speech/language pathology services to patients in the home or facility
- Participate in developing and reviewing treatment plans
- Assess and treat speech, voice, language, hearing and swallowing disorders
- Administer and interpret diagnostic tests and apply therapeutic treatments
- Record and report changes in patient condition and response to treatment
- Provide instruction and training on alternative communication systems when appropriate
- Maintain and submit documentation as required by company policy
- Collaborate with care coordination teams and participate in discharge planning
- Ensure confidentiality of patient information and maintain professional conduct
You Will Be Required To Have:
- Certification of Clinical Competence in Speech-Language Pathology or Audiology granted by ASHA
- Minimum six months experience as a speech therapist/speech-language pathologist
- Home Health experience is a plus
- Current and unrestricted license
- Current CPR certification
- Good organizational and communication skills
