The Cancer Care Coordinator supports patient centered care by serving as a single point of contact for patients, families, referring physicians and caregivers to promote timely access to care, including guiding patients, families, and caregivers on the patient's cancer care plan. Functions autonomously and in collaboration with multiple health care providers as a patient advocate and educator. Effectively coordinates care throughout the disease continuum and across multiple settings by utilizing resources effectively and efficiently, promoting a multidisciplinary team concept. Supports and implements patient safety and other safety practices as appropriate. Maintains professional competencies by attending educational programs, participates in professional organizations, subscribing to reading professional journals or other appropriate activities. Each care coordinator will be assigned to a provider, working as a team to provide high quality cancer care to patients and their families.
Schedule :
- 80 hours every 2 weeks
- Monday-Friday
- 7 : 30AM-4PM
Pay and Benefits :
Associate / Diploma in nursing starting at $36.08 per hour
