What are the responsibilities and job description for the Manager PACE Home Care position at Centra Medical Group?
Manager Home Care is responsible for the development, implementation, coordination, and management of home-based personal care services for PACE program participants. This position will provide direct and indirect supervision to all home care certified nursing assistants. This position acts as the leader in collaborating with members of the interdisciplinary team, providing the foundation for creating the interdisciplinary plan of care. He/She works to promote and sustain a culture reflective of Centra's best practices for quality and service, creates and maintains a safe home care environment, and contributes to a healthy work environment.
Required Qualifications:
- Graduate of an accredited nursing program with a current Virginia or multistate nursing license
- Two (2) to five (5) years of nursing experience with at least two (2) years of supervisory experience and one (1) year of caring for the frail or elderly.
- Hold a current, active, American Heart Association Basic Life Support (AHA BLS) course completion card.
Preferred Qualifications:
- Bachelor's of Science Degree in Nursing (BSN)
- Two (2) years of nursing experience in home health
Essential Duties and Responsibilities:
- Assesses the home care needs of frail elderly participants and develops specific plans of care to address identified needs
- Participates in the development of the participant plans of care, the coordination of a 24-hour care delivery system, and the interdisciplinary team (IDT) plan of care
- Coordinates the implementation of home care services, including personal care, homecare-chore services, and ensures quality services are provided according to the needs of individual participants
- Supervises all contracted care with outside CNA/PCA agencies, if utilized, to include: orienting, scheduling, audits, drafting and documenting all plans of care to be implemented, conducting supervisory visits and working with the business office to ensure billing is correct.
- Serves as a coach, educator, and resource to home care certified nursing assistant staff by performing joint home visits to promote development of staff
- Counsels home care certified nursing assistants when expectations and policies are not being followed correctly. Provides follow up to staff after counseling. Documents in employee files appropriately
- Performs human resource management of all home care staff in association with the Director of Nursing (PACE) and/or Site Director of their respective PACE site
- Uses established processes and electronic systems to approve requests for compensation and expense reimbursement
- Provides appropriate instruction and direction to home care certified nursing assistant staff
- Participates in the PACE management team and serves as liaison between administration and in home certified nursing assistants
- Can conduct initial and regulatory assessments of participant needs
- Communicates participant changes to team members
- Completes all documentation in the medical record as required
- Investigates complaints and incidents in a timely manner and resolves issues, as able Meets a standardized set of competencies for the specific position established by Centra PACE and approved by Centers for Medicare and Medicaid Services (CMS) before working independently
