Referral Coordinator Outpatient Rehabilitation

The Referral Coordinator is responsible for managing all Centra and non-Centra referrals for Outpatient Physical, Occupational, and Speech therapies. This role is responsible for daily operations of the Outpatient Rehab Admin Hub. The referral coordinator obtains all necessary information to facilitate access to therapy services.


Required Qualifications:
High School Diploma / GED
Minimum of three (2) years of experience working in a medical office environment.


Preferred Qualifications:
Associates Degree in Medical Office Management, Business, or a related field
Knowledge of rehab office and revenue cycle processes.


Receives all Centra referrals for outpatient OT/PT/SLP and routes them both internally and externally when requested.
Receives and manages all external referrals to PT/OT/SLP.
Coordinates the daily operations of the Admin team to prioritize and assign tasks.

Works along with team to perform the following:
Expedites access to care by managing the centralized scheduling phone and fax
Guides the patient as to which clinic can meet their geographical and clinical needs, scheduling as appropriate and maintaining communication with the clinics to provide great customer service for the patient.
Works collaboratively with hospital discharge planners, referral coordinators, and others in seeking out all information pertinent to referral and patient information needs.
Scheduling of initial evaluations, intakes patient information (i.e. demographics, insurance information, and patient history) and enters data into electronic medical record.
Obtains pre-authorization and patient benefits from insurance companies to determine eligibility of services.
Follows through with customer service issues until resolved.

Leads daily cross clinic morning huddle.
Assists practice leadership with the implementation of process improvement activities.
Tracks and reports out on daily, weekly, and monthly key performance indicators (KPI’s) to ensure efficiency of team, add or flex staffing based on needs, and troubleshoot any issues.
Ensures compliance with organizational and departmental policies and procedures as well as federal and state regulatory requirements

Other duties as assigned.
Provides general support staff oversight in the absence of other management.
Provides orientation and training for admin staff members.
Coaches employees regarding KPI expectations

Travel Required
Travel is expected to be less than 5% of the time in times of cross clinic coverage needs.
