What are the responsibilities and job description for the RN Mental Health Intake Coordinator-LGH-Limbo position at Centra Medical Group?
The RN Mental Health Intake Coordinator provides telephone and data collection support for admissions to mental health programs and referral information to in-house and outside resources. Handles and directs calls and website requests from referral sources and the general public. Updates and maintains database and generates reports. Works within resource constraints and individual patient needs within transfer process and coordinates with the hospital system to prioritize and mobilize patients to secure the best possible behavioral health service placement.
Required Education: Associate degree or higher from accredited School of Nursing
Required Experience: 3 years of nursing experience. Exceptional verbal, written, and telephone communication skills. Maintain good interdisciplinary and community-based communication skills and relationships. Demonstrated leadership and problem-solving skills. Computer skills in various software programs designated by department head. Equipment knowledge of computers, printers, fax machines, copiers, and telephones.
Required Certifications and Licensures: Current Virginia or Multistate Nursing Registration. Hold a current, active American Heart Association Basic Life Support (AHA BLS) course completion card.
Preferred Education: Bachelor of Science Degree in Nursing (BSN)
Preferred Experience: Background in psychiatric nursing.
Coordinate with interdisciplinary team to help place patients in appropriate care setting.
Responsible for continuity of operations within the facilities by overseeing all patient admissions and transfer activity while monitoring capacity both currently and prospectively.
Communicate with interdisciplinary team to coordinate patient flow.
EMR Documentation as appropriate
Review EMR for appropriate placement and accommodations
Utilizes the nursing process as a frame of reference for the placement of patients for the most appropriate transfer
Demonstrates the necessary knowledge and skills to address the care needs appropriate to age, spiritual and cultural needs of the population served
Collaborates with the unit Charge Nurse, Clinical Coordinator, or UM to gather issues and concerns associated with bed constraints within our hospitals
Serves as a resource for interpretation of policies and procedures as related to patient placement including, but not limited to placements of isolation or special care patients
Provides bed availability information to reporting bodies when necessary
Assists transfer center with pre-admit documentation
Advises appropriate staff of situations requiring follow-up attention
Answers all calls and records on computerized forms/database
Assists lead marketing specialist and marketing director as required
Communicates information with physicians and others in a timely manner
Completes continuing education requirements as mandated by Centra policy
Demonstrates excellent written and verbal communication skills
Demonstrates flexibility to changing workloads and good judgement in handling difficult situations with a positive attitude
Establishes and maintains positive relationships with providers, referral sources and other Centra departments
Facilitates admissions to acute hospitalization programs
Identifies quality or customer service issues and addresses appropriately and in a timely manner
Maintains a clean and organized workspace
Maintains confidentiality
Participates in pertinent workshops/training sessions
Provides data and reports/charts when scheduled and upon request
Provides follow-up with physicians and referral sources in a timely manner
Provides information and refers non-admission calls/inquiries to appropriate resources
Records accurately all calls and inquiries on computer database
Updates database and generates reports for tracking and marketing purposes
Verifies insurance coverage and obtains necessary authorizations
Works cooperatively with supervisor, director, providers and referral sources
Actively participates in staff meetings
Performs other duties as assigned.
