Find your purpose working as a Social Worker with CentraCare! As a member of the interdisciplinary team, the Social Worker will use a care process to evaluate and determine appropriate needs for patients across the health care continuum. The use of independent judgment and collaboration to promote the safety and welfare of our patients will be used. Actions of the Social Worker will demonstrate support of Patient and Family Centered Care Principals and will ensure resources are used appropriately and efficiently. This may include provision of direct services or education to other members of the health care team. Anticipatory judgment may be needed to proactively address patient needs. The Social Worker will remain active in associated professional activities that may require community involvement.
Schedule
- Full-time | 72 hours every two weeks
- Monday - Friday | 8-hour shifts between 7:30 am - 5:00 pm
- Hybrid position including approximately 50% of time spent working from home as well as 50% of time on-site at Long Prairie, Melrose & Sauk Centre Primary Care Clinics
Pay and Benefits
- Starting pay begins at $26.59 per hour and increases with experience
- Pay range: $26.59 - $39.88 per hour
- $3,000 hiring bonus and $5,000 relocation assistance for eligible candidates

Qualifications
- Bachelor's Degree in Social Work or related field from an accredited college
- Current Licensed Social Worker (LSW) in Minnesota
- Experience with the diversity of the local community is preferred
- Must have a working understanding of medical terminology and electronic medical records; Epic experience is preferred
To be eligible for the hiring bonus and relocation assistance, you must be an external candidate who has not worked for CentraCare for at least 9 months at the date of hire and be hired into an eligible full-time or part-time benefited position. Relocation assistance also requires that you currently live more than 50 miles from your primary work location and move within 30 miles of it within 120 days of employment. CentraCare pay promotions to include but not limited to, hiring bonus and relocation assistance may be discontinued at any time due to business needs.
