Description: We are seeking a skilled Social Worker to join our interdisciplinary team at CentraCare. As a Social Worker, you will use a care process to evaluate and determine appropriate needs for patients across the healthcare continuum. Key Responsibilities:
- Evaluate patient needs and develop plans to address them
- Collaborate with the healthcare team to provide patient-centered care
- Provide education to patients and families on health-related topics
- Stay up-to-date with community involvement and professional activities
Schedule:
- Full-time position, 72 hours every two weeks
- Monday - Friday, 8-hour shifts between 7:30 am - 5:00 pm
- Hybrid position, 50% remote work and 50% on-site at Long Prairie, Melrose & Sauk Centre Primary Care Clinics
Qualifications:
- Bachelor's Degree in Social Work or related field from an accredited college
- Current Licensed Social Worker (LSW) in Minnesota
- Experience working with diverse populations preferred
- Working understanding of medical terminology and electronic medical records required
