HOW YOU’LL MAKE AN IMPACT
The Care Coordinator is responsible for coordinating the care for individuals and/or families receiving services, promoting quality care, and supporting collaboration for Certified Community Behavioral Health Care (CCBHC) clients.
- Promote recovery and psychiatric stability using established rehabilitative principles and best practices.
- Provide care coordination for clients as needed, coordinating with referral source to assess best services for client.
- Assess, refer, coordinate, monitor services and communicate regularly with all providers involved to ensure appropriate care, reduce duplication of services and promote client autonomy.
- Assist clients in addressing barriers to accessing services, including transportation, insurance, finances, and legal issues.
- Maintain current knowledge of mental health service system with the ability to locate information and resources for a variety of clients needs, including medical, housing, employment, social services, and community programs.
- .Assist support staff and clinicians in identifying appropriate referrals and resources for clients or potential clients.
- Attend weekly team meetings in assigned location to collaborate with providers.
