How You’ll Make An Impact
Mental Health TCM provides home and community-based mental health services to adults with severe and persistent mental illness (SPMI), and children with severe emotional disturbance (SED) and their families.
WHAT YOU’LL NEED
The Targeted Case Manager shall meet the qualifications defined in Minnesota Statute 245.462 subdivision 4:
COMPENSATION
Mental Health Practitioner: $20.50 - $23.50
Mental Health Practitioner Bachelor's Degree: $22.50 - $25.50
Mental Health Practitioner Master's Degree: $24.50 - $27.50
(Commensurate with experience and degree in behavioral sciences or related fields)
SCHEDULE
Full Time: Monday-Friday ability to choose your hours between 7:00 AM – 6:00 PM

How You’ll Make An Impact
Mental Health TCM provides home and community-based mental health services to adults with severe and persistent mental illness (SPMI), and children with severe emotional disturbance (SED) and their families.
- Establish relationships with assigned clients and their supports and meet with each client as planned.
- Assess, refer, coordinate, monitor services and communicate regularly with all providers involved to ensure appropriate care, reduce duplication of services, and promote client autonomy.
- Be familiar with implementation of health integration, including care coordination agency wide.
- Maintain current knowledge of the mental health service system with the ability to location information and resources for a variety of client needs, including medical, housing, employment, social services, and community programs.
- At a minimum meet the monthly required contact for each client on caseload; document all services provided accurately, completely and within required deadlines.
WHAT YOU’LL NEED
The Targeted Case Manager shall meet the qualifications defined in Minnesota Statute 245.462 subdivision 4:
- Be a mental health practitioner as defined in section 245I.04, subdivision 4, or have a Bachelor’s degree in one of the behavioral sciences or related fields including but not limited to, social work, psychology, or nursing from an accredited college or university.
- Complete initial and ongoing training requirements defined for the position to meet required standards.
- Clear DHS background study.
- Valid Driver’s License and acceptable driving record.
- Experience with or providing direct care to an individual with Co-Occurring Substance Disorder.
