Job Description :
- Engage individuals experiencing homelessness on streets, subways, and parks, conducting medical, mental health, and safety assessments.
- Encourage individuals experiencing street homelessness to seek shelter, working with crisis coordinators and outreach teams to build trust and relationships.
- Determine if individuals have a mental illness or medical condition necessitating immediate care and treatment in a hospital setting.
- Collaborate with crisis coordinators and outreach workers in conducting street-level mental crisis assessments.
- Evaluate mental health conditions to determine risk levels of harm to self or others.
- Assess individuals for homicidal or violent behaviors that place others at risk.
- Coordinate with law enforcement for the transportation of individuals in mental crisis to appropriate psychiatric facilities if necessary.
- Collaborate with outreach teams to ensure continuum of care and proper discharge planning.
- Provide medical attention to individuals pursuant to mental hygiene laws.
- Initiate processes for the removal of individuals to designated psychiatric facilities when necessary.
- Document and assess individuals experiencing homelessness, both in the field and office settings, using appropriate forms.
- Confer with hospital and psychiatric clinicians regarding assessments and recommended treatment plans.
- Obtain approval to operate as a clinician within a mobile crisis outreach team.
- Maintain and update records in a timely manner.
