What are the responsibilities and job description for the Pediatric Care Coordinator position at CENTRAL OREGON PEDIATRIC ASSOCIATES LLP?
Central Oregon Pediatric Associates (COPA) of Bend, Oregon, the largest pediatric practice in the region, is hiring a full-time Pediatric Care Coordinator.
This is a Hybrid position, Monday – Friday, 8:30am – 5:00pm.
At COPA, the Pediatric Care Coordinator plays an integral role serving patients at our friendly and welcoming medical clinic for children of all ages. The Pediatric Care Coordinator is an integral member of the Patient-Centered Primary Care Home (PCPCH)and provides evidence-based support to pediatric patients with medical and social complexities. The Care Coordinator works directly with Lead RNs to manage all non-clinical components of care for COPA’s most complex and vulnerable panel of patients. The Care Coordinator’s primary function is to work with care team members, other members of the treatment team, social service agencies, public agencies, and health insurance companies to coordinate care and services needed to assist patients in achieving optimal health and meeting goals set by their care plan.
The role of the Pediatric Care Coordinator will include but is not limited to the following duties:
Maintains an in-depth knowledge of principles, practices, standards, and techniques and demonstrates knowledge in accordance with COPA policies and procedures within pertinent laws and regulations in the following areas:
- Creates and manages a Patient Care List of all patients referred through EPIC
- Documents all communication and support with patient care in a Telephone Encounter note in EPIC, as Coordination of Care.
- Manages Epic Care Coordination Pool, Care Coordination group email, Care Coordination and Work Queue
- Assist families in understanding the WRAP referral process and completes applications for submission.
- Attends WRAP meetings as COPAs representative to ensure medical needs are met for the patient and assists in meeting goals.
- Attends Care Coordination meetings as a COPA representative to ensure medical needs are being met for the patient and assisting in meeting goals.
- Point of contact on the Care Coordination Team with Pedal clinic, County Nurses, Schools, etc to communicate concerns or needs for the patient to help meet patient needs.
- Supports Community Health Worker in maintaining knowledge of available community resources and can identify appropriate referral patterns to remove barriers due to various social determinants of health.
- Makes follow up calls on referrals and assists families as needed with scheduling specialty appointments.
- Developing/managing tracking and documentation systems for patient care coordination and care management activities across the continuum, including care transitions, referrals, report management, and two-way communication between the PCP, specialists, and or other providers.
- Integration of the patient/family into care coordination and care management planning and communications, assuring that the patient/family are informed and supported in decision-making.
- Utilizes HIE as required for quality metrics and patient care.
- Facilitates communication between the patient, physicians, and other team members.
- Facilitates paperwork completion by families for treatment needs.
- Works cooperatively with other members of the treatment team, social service agencies and public agencies.
- Provides letters of medical necessity to obtain equipment or services, and prepares prescriptions for DME, formula and related equipment to be reviewed and signed by the primary care provider.
- Supports Referral Team in assisting families when filing grievance and appeals for services or equipment that has been denied.
- Participates in the development of patient education materials.
- Coordinates care with specialty pediatric physicians performing outreach services at COPA.
- Supports training of other staff in the coordination of care with specialists and other providers across the continuum.
- Provides support to other Care Coordination team members as needed.
- Member of the PCPCH Care Coordination team and works to ensure compliance with processes, documentation and attestation requirements directly tied to the PCPCH Care Coordination Standards.
- Works closely with the State's Early Hearing Detection Program by following up on all referred newborn hearing screen results and facilitating any required follow up. –
- Supports Community Health Worker in maintaining knowledge of available community resources and can identify appropriate referral patterns to remove barriers due to various social determinants of health.
- Perform other duties as assigned.
- Execute care coordination elements of the care plan created by clinical team.
- Establishes relationships as liaison with health plan case managers as indicated.
- Work with community partners to streamline processes and make it easier to navigate for our patients.
- Provides the administrative support for our internal Allergy department including patient scheduling and obtaining the necessary prior authorizations. This is not limited to Allergy and will include other panels of patients as we identify them.
Qualifications
Education and Certification:
- Bachelors’ degree in related healthcare field preferred.
- Certified Case Manager (CCM), Case Manager Certified (CMC), Certified Professional in Health Care Quality (CPHCQ), or Certified CHW preferred.
Experience:
- Minimum of three (3) years healthcare experience required; pediatric or related specialty experience preferred. Experience in case management, disease management, or care coordination preferred.
- Bilingual in Spanish and English a plus.
- Exceptional and proven communication skills and ability to interact with a diverse population of patients, families and professionals. Ability to facilitate and reach consensus among health care professionals and community partners to achieve expected results. Ability to work both independently and in a team to promote team driven outcomes. Clinical informatics knowledge and skills. Experience giving presentations across multiple platforms, proficient with Microsoft Office Suite. Epic EMR experience preferred.
Physical Requirements:
- Use of clear and audible speaking voice and the ability to hear normal speech level in person, over the telephone or through use of other required technology. Sitting or standing for long periods of time. Frequent repetitive motion includes computer keyboard use, reaching with hands and arms, and walking. Occasional lifting and or moving up to 30 pounds and on rare occasions up to 50 pounds. Specific vision abilities required include viewing computer monitor for long periods of time, close vision, distance vision, color vision, peripheral vision, and depth perception.
- Must be able to wear appropriate personal protective equipment (PPE) as required.
Working Conditions:
- Hybrid- May be eligible for combination of on-site and home office within Central Oregon; as job requirements allow. Refer to Local Remote Work Policy for details.
- Travel between COPA clinics, business office and community partners as required.
