Education and Experience:

Must have one of the following: Bachelor's degree in social, behavior or human services field from an accredited university or;
- Associate degree in a social, behavioral, human service, or health related field or:
- High school diploma/GED and 2 years of paid or unpaid experience with individuals with Intellectual or Developmental Disabilities
- Must have valid TX driver's license and be insurable
Job Purpose:
- Provides Service Coordination to individuals such as preventing, and managing crisis situations
- Performs assessments, monitoring, assessing needs
- Service planning and coordination
Job Function:
- Maintains regular contact with individuals served to monitor services and identify needs
- Service Coordinator assessments will be completed initially, annual, and/or as needed for all individuals served
- Will identify gaps in services so individual served experiences a continuum of care to the greatest extent possible
- Develops Initial and Annual Person Directed Plan and makes revision based on the needs of the individual
