Education and Experience:

Must have one of the following:
- Bachelor's degree in social, behavior or human services field from an accredited university; or
- Associate degree in social, behavior, human service or health-related field; or
- High school diploma/GED with 2 years of paid or unpaid experience with individuals with Intellectual Developmental Disabilities
- Must be able to qualify as a Qualified Intellectual Disabilities Professional (QIDP)
- Preferred to have at least one (1) year of experience in working with individuals with Intellectual and/or Developmental Disabilities
- Must have valid TX driver's license and be insurable
Job Purpose:
- Performs Service Coordination to individuals such as preventing, and managing crisis situations
- Performs assessments, monitors, assessing needs, service planning and coordinating and making appropriate referrals
- Monitor and evaluate the effectiveness of care plans and make adjustments as necessary.
- Maintain accurate and up-to-date client records and documentation.
Job Function:
- Maintains regular contact with individuals served to monitor services and identify needs
- Will identify gaps in services so individual served experiences a continuum of care to the greatest extent possible
- Develops Initial and Annual Person Directed Plan and makes revision based on the needs of the individual
