What are the responsibilities and job description for the PACE Outreach Specialist (Central Valley PACE - Merced) position at Central Valley PACE?
The Outreach Specialist plans, organizes, and directs all outreach and marketing efforts. Involved in the intake and enrollment, and coordination with the interdisciplinary team to facilitate new PACE participant enrollment.
Schedule is Monday – Friday, working from 8:00am – 5:00pm.
Compensation:
$25.87 - $27.16 an hour

Duties and Responsibilities
- Plans and organizes outreach and marketing efforts, intake and enrollment, and coordination with the Interdisciplinary team to facilitate enrollment.
- Coordinates and executes intake and enrollment activities and processes as required.
- Identifies opportunities to collaborate with community organizations conducive to gaining eligible PACE participants.
- Establishes rapport and maintains relationships with community leaders and partners including physicians, hospital discharge planners, post-acute facilities and other potential referral sources in the service area.
- Develops and implements annual marketing plan to achieve enrollment goals.
- Provides education referral sources and educates the community on PACE services.
- Participates in quality improvement.
- Participates in public relations to support goals including television, radio, press, and public speaking opportunities,
- Answers calls from prospective participants and explains the Central Valley PACE enrollment process.
- Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties
- Schedules intake appointments for enrollment team and, when needed, conduct intake appointments.
- Conduct tours of the program and serve as a host for prospective participants and families
- Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
- Other projects and duties as assigned.
Physical Demands
- Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.
- Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
- Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
- Must have high manual dexterity.
- Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
Work Environment
- Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
- Subject to unpleasant odors.
- The noise level is usually quiet to moderate, but may at times be noisy and crowded.
Education/Experience Requirements
Minimum Qualifications
- Valid California Driver’s License, acceptable driving record and vehicle insurance.
- Highly motivated, team player, self-directed, able to execute tasks in a quickly changing environment.
- Strong customer service orientation.
- Deep commitment to helping unlock the potential of our most vulnerable seniors.
- Fluency in spoken and written English and other language, Spanish and Hmong preferred.
- Travel to other locations as requested.
Education/Experience
- Associate’s Degree in Business or related field required; or two (2) years of work related experience in lieu of degree.
- A minimum of two (2) years marketing, community outreach, advocacy in government, or human services assistance programs experience in the healthcare industry or related field.
- Current BLS CPR Card certified by the American Heart Association
