Job Title : Physical Therapist - Rehabilitation Therapy
Job Specialty : Physical Therapy
Job Duration : 13 weeks
Shift : Tuesday - Saturday. First week will be Monday-Friday for orientation
Guaranteed Hours : 36 hours
Experience : Minimum 1 year
License : Vermont PT licensure
Certifications : Home Health experience and OASIS experience required
Must-Have : McKesson / Netsmart EMR experience
Job Description :
- Provide one-on-one therapy care for patients in their home setting
- Assess individual conditions and develop personalized care plans to enhance mobility and independence
- Collaborate with interdisciplinary teams to meet patient needs
- Perform standard therapy measurements such as ROM, strength, and balance tests
- Use therapeutic modalities to optimize patient care
