Your Role:
You will make home visits to clients, perform diagnostic tests and measurements, develop and implement individualized care plans, educate and instruct clients and family members, accurately document observations and interventions, and continually assess and revise care plans.
Qualifications:
To be successful in this role, you will need a current Physical Therapist license in the state, a minimum of one year of recent work experience as a Physical Therapist, graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, and solid computer skills.
