Work with clients in their homes, developing and implementing individualized care plans that address their unique needs and goals.
Perform diagnostic tests and measurements, such as mobility assessments and strength evaluations, to inform treatment plans.
Design and implement therapeutic exercises, gait training, and other interventions to promote client recovery and independence.
Collaborate with multidisciplinary teams to ensure comprehensive care and coordination.
Accurately document observations, interventions, and evaluations to support client care management.

**Qualifications:
A current Physical Therapist license in the state
A minimum of one year of recent work experience as a Physical Therapist
Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
Solid computer skills and prior experience with electronic medical records (EMR) preferred
