Position Requirements
- Acute Care Nursing experience: 5 years - Required
- Current/recent Case Management experience: 1 year - Required
- Current/recent Utilization Review and/or Discharge Planning experience: 1 year - Required
- Demonstrated knowledge of MS Office (Outlook, Excel) - Required
- Prior travel experience: Highly preferred
- Case Management Certification - Required
- ASN - Required
- BSN – Preferred
- RN License for State of New Hampshire or Compact - Required
- BLS Certification from AHA - Required
- Case Management Certification – Required
- The UR RN/Case Manager responsibilities
- Accountable for a designated caseload and plans effectively to manage the length of stay, promote efficient utilization of resources and facilitate payer authorization.
- Facilitates collaborative management of patient care, intervenes as necessary to remove barriers to efficient care delivery,
- Applies process improvement methodologies in evaluating the outcomes of care coordination
- Supports and coaches clinical documentation by staff and providers ensuring documentation reflects the severity of illness and intensity of service.
