Employment Type : Fulltime Travel Contract Temporary
Contract Length : 13 weeks contract Accepting Local and Travel Candidates
Pay Rate : Range $2320 to $2400 Weekly Gross pay (Nontaxable travel / housing and M&I stipend available for qualified / eligible travelers)
Mileage Reimbursement : $0.50 / mile for distance covered between patient home appointments
Unit : Home Health
Shift : 8 Hours; Days 8.00a 5.00p
Schedule : 40 hours / week guaranteed five shifts per week MonFri
Position Requirements :
Current Physical Therapist experience : 1 2 years Required
Recent / Current Home Health PT experience of at least 1 year : Highly preferred
Education License and Certification Requirements :
Bachelor of Science in Physical Therapy or Similar Required
Current State License as Physical Therapist or Compact if applicable Required
BLS Certification from AHA or American Red Cross Required
Job Description :
Provide skilled physical therapy services to patients in their homes as ordered by the attending physician.
Observe record and report patient reactions changes and progress to IDT team
Treats patient to relieve pain develop or restore function and maintain maximum performance using physical means such as exercise massage heat water etc.
Counsel patient and family on the disease / injury and how to manage in all phases of physical therapy.
Instruct patient and family members on proper use of equipment such as wheelchairs braces walkers crutches canes and other prosthetic / orthotic devices.
Oversee and supervise therapy services of patient provided by PTAs and when appropriate nurse assistants.
Communicate with the PTAs and nurse aides on supervisory visit results.
Completes all clinical documentation following agency protocol and Medicare / Federal guidelines.
Must meet 30 productivity points per week
Territories to cover : Kettering OH and surrounding areas
