Century Ridge is seeking a Wellness Coordinator to join our team. The Wellness Coordinator is responsible for coordinating the delivery of health and wellness services in accordance with resident needs and company policies and procedures. The ideal candidate will have knowledge of regulatory requirements for compliance in a CBRF, experience in caring for frail older adults and excellent organizational and communication skills. Full and part-time candidates will be considered.
GENERAL RESPONSIBILITIES
Maintain high standards of resident care and customer satisfaction.
Build relationships with residents and their families through good communication and follow-through. Work with partner providers to develop a collaborative working relationship. Participate in training and supervision of care staff.
Review 24-hour report and wellness monitoring, following up on noted concerns, completing current and accurate documentation and communication to physicians and responsible parties.
Respond quickly to all resident changes in condition and emergencies, process incident reports and ensure proper handling of resident falls and elopement attempts and completion of risk assessments and interventions.
Participate in the development of service plans and interventions and communicate services and support required to meet resident needs.
Help facilitate and coordinate details related to new admissions and hospital returns.
Assist in coordinating resident dietary and ancillary service needs.
Participate in on -call rotation.
QUALIFICATIONS
Excellent communication skills
Ability to creatively problem-solve in both resident care and employee management situations
Must be a team player, well organized and flexible
EDUCATION and / or EXPERIENCE
2 plus years in assisted living and / or long term care
Supervisory / management experience a plus
Associate degree or higher in a health- related field, LPN or MA preferred
