LOCATION: Ceres HQ/ Remote, and across Ceres sites.

EXPECTED HOURS: 30-40 Hours/Week

Position Summary

The Registered Dietitian Nutritionist Care Coordinator is responsible for supporting the coordination of nutrition care services for Ceres’ clients. Working under the guidance of the Nutrition Care Manager, the RDN provides nutrition services to clients, including assessing nutritional needs and developing individualized care plans aimed at improving health outcomes for clients managing chronic and complex medical conditions. The RDN will follow the Nutrition Care Process (NCP) to provide nutrition counseling, medical nutrition therapy (MNT), and education through telephonic, virtual, or in-person consultations. S/he/they must be comfortable presenting nutrition information to both individuals and groups, as well as with a wide range of people, personalities, and cultures. This position is one that is client and community facing; therefore, it requires staff to be warm, compassionate, emotionally mature, organized with excellent follow through and reliability, and able to meet a client in one of the most difficult moments of their lives.

Essential Functions And Responsibilities

  • Coordinates the nutrition care process for Ceres’ clients, including conducting nutrition screening, individual 1:1 nutrition assessment, counseling, education, and surveys as required by funding and contracts.
  • Conducts nutrition screening for eligibility as required by referrals from the client care team.
  • Supports contract and grant requirements including educational material development, nutrition education via virtual or in-person classes, 1:1 visits, and documentation of activities related to the contract or grant.
  • Collaborates and supports the Nutrition Care Manager/RDN in development and maintenance of Medically Tailored Meal guidelines and meal plans to ensure compliance with FIMC Accreditation requirements and to ensure efficacy or the intervention. This may include performing kitchen and meal preparation audits and/or recipe and menu substitution approval.
  • Assists in maintaining recipes and nutrient analysis in Ceres’ recipe database and works collaboratively with Culinary and Nutrition Care staff to ensure that all recipes are clear, accurate and meet established MTM nutrition guidelines. Conducts nutritional analysis either under the oversight of or as an RDN.
  • Maintains case notes and other required tracking and documentation in Salesforce for all client interactions, including nutritional assessments and medical nutrition therapy sessions.
  • Develops and delivers virtual and in person training in health, wellness, and nutrition to the community at large, staff, youth, and clients.
  • Conducts presentations and outreach, representing Ceres in the community.
  • Develops and keeps current nutritional education and resources including Nutrition Bites and curricula used for medical nutrition therapy.
  • Performs administrative tasks such as database reporting, emails, mailings and preparing packets of program materials.
  • Follows up with referral partners when required or requested.
  • Protects client data according to HIPAA standards.
  • Participates in internal and external meetings as required and necessary.
  • Other duties as required or requested.

Qualifications

  • Bilingual in Spanish preferred, multiple language skills a plus.
  • Registered Dietitian/Registered Dietitian Nutritionist.
  • Minimum of 2-3 years of experience providing Medical Nutrition Therapy for chronic diseases such as diabetes, cardiovascular disease and kidney disease.
  • Possess a strong knowledge of the Nutrition Care Process and ADIME documentation format.
  • Valid California driver’s license and excellent driving record.
  • Proficiency in MS Office Suite, Salesforce experience strongly preferred.
  • Ability to listen, communicate warmly and clearly, and set clear boundaries.
  • Excellent communication skills both verbally and in writing; able to communicate complex nutrition information simply and effectively; comfortable presenting to groups, both in person and virtually.
  • Strong organizational skills and attention to detail, including the ability to implement projects according to a timeline.
  • Self-directed; comfortable working in a fast-paced collaborative environment.
  • Self-aware with an ability to self-assess.
  • Must be comfortable working with people with serious illness.
  • Cooking, recipe development and food preparation experience a plus.

  • While performing the duties of this job, the employee is regularly required to stand or sit at a workstation; use hands and fingers to operate a computer keyboard and mouse; talk and hear to communicate with coworkers.
  • Light to moderate lifting of up to 30 pounds, bending, stooping and climbing a ladder is occasionally required.
  • The work environment is primarily indoors with the majority of time spent sitting at a desk, on the phone or a computer, with moderate noise (office equipment, conversation from adjacent coworkers and commercial kitchen equipment) in the background.
  • Must be able to sit or stand at a computer terminal for an extended period of time.
