Job Summary:
The Pediatric and Adolescent Transformative Health (PATH) Clinic is a 12-member collaborative that provides innovative structural mental and developmental health services to children, adolescents, and families on the west and southwest sides of Chicago. PATH is seeking to hire a Pediatric Care Coordinator for Behavioral Health and
Developmental Therapy which will play a critical role in supporting children and their families throughout the continuum of care. This position is responsible for coordinating services, resources, and support for pediatric
and adolescent patients with behavioral health needs and developmental challenges. The Care Coordinator will collaborate with multidisciplinary teams, including healthcare providers, therapists, schools, and community organizations, to ensure a comprehensive and integrated approach to care. Key Responsibilities:
- Serve as the primary point of contact for families and children receiving behavioral health and developmental therapy services.
- Develop, implement, and monitor individualized care plans in collaboration with the healthcare team, patients, and families.
- Facilitate communication between families and healthcare providers to ensure that all parties are informed and involved in the care process.
- Conduct comprehensive assessments to identify the behavioral health and developmental needs of pediatric patients.
- Coordinate referrals to appropriate services, including therapy, counseling, educational support, and community resources.
- Assist families in navigating complex healthcare systems and accessing available resources.
- Provide support and education to families regarding behavioral health and developmental issues, treatment options, and available resources.
- Facilitate family meetings and support groups to promote engagement and collaboration among caregivers.
- Act as an advocate for families, ensuring their voices are heard in treatment planning and decision-making processes.
- Collaborate with multidisciplinary teams, including pediatricians, mental health professionals, occupational and speech therapists, and educators, to ensure coordinated care.
- Maintain accurate and up-to-date records of patient assessments, care plans, and progress notes in compliance with organizational policies and regulations.
- Monitor and evaluate the effectiveness of care plans and interventions, making adjustments as needed based on patient outcomes.
- Prepare reports and summaries for internal and external stakeholders to track program effectiveness and identify areas for improvement.
- Participate in quality improvement initiatives to enhance the delivery of pediatric behavioral health and developmental services.
- Stay informed about best practices, evidence-based interventions, and relevant policies in pediatric behavioral health and developmental therapy.
- Engage in professional development opportunities to enhance skills and knowledge in care coordination and pediatric behavioral health.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Nursing, or a related field.
- Previous experience in pediatric and adolescent care coordination, behavioral health, or developmental therapy settings is highly desirable.
- Strong understanding of pediatric and adolescent behavioral health and developmental challenges, including knowledge of evidence-based practices and treatment modalities.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work collaboratively with diverse populations, including children, families, and multidisciplinary teams.
- Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
- Certification in care coordination or related fields is a plus.
Working Conditions:
