CHANGE Inc. is seeking a Full Time Patient Care Coordinator for Family Medical Care. Applicant must have an active RN, LPN, or CMA (with appropriate state licensure and schooling). Applicant will be coordinating continuation of patient care with a Family Medical Care Provider and assisting the medical team with any other duties deemed necessary. Applicant must have strong leadership skills and the ability to lead a team, excellent communication and organizational skills, medical office experience, EMR experience and the ability to grasp new technology quickly. A valid driver’s license is required. Applicant would need to pass a pre-employment background check and drug screening. EOE Closing date for this position is February 14th, 2025.
