Job Summary:
We are seeking a compassionate and committed Faith-Based Drug and Alcohol Rehab Case Manger to join our team. In this role, you will support individuals recovering from substance use disorders through a holistic approach that integrates faith, spirituality, and evidence-based practices. You will work closely with clients to develop and implement personalized care plans that promote recovery, emotional well-being, and spiritual growth.
Key Responsibilities:
- Conduct thorough assessments to evaluate clients’ substance use history, mental health, and spiritual needs and exit strategies.
- Develop individualized rehabilitation plans in collaboration with clients, families, and the treatment team, incorporating faith-based elements.
- Facilitate access to community resources, including counseling, medical care, vocational training, and support groups.
- Provide ongoing support and encouragement, helping clients to overcome obstacles and stay committed to their recovery journey.
- Lead or co-facilitate faith-based support groups and activities that promote spiritual growth and community engagement.
- Monitor clients’ progress and adjust care plans as needed, offering crisis intervention and support when necessary.
- Advocate for clients’ rights and provide education on recovery resources and options.
- Maintain detailed and accurate documentation of client interactions and progress, ensuring compliance with regulatory standards.
- Collaborate with clergy, spiritual leaders, and other community partners to enhance clients’ spiritual support systems.
- Stay informed about best practices in addiction recovery and faith-based interventions.
Qualifications:
- Specializing in the field of Social Work, Counseling, or a Peer Support Specialist is preferable.
- Previous experience in case management, substance abuse treatment, or a related field.
- Strong understanding of addiction recovery processes and faith-based approaches to healing.
- Excellent communication and interpersonal skills, with the ability to connect with clients from diverse backgrounds.
- Strong organizational skills and attention to detail.
- Ability to work independently and as part of a multidisciplinary team.
- Familiarity with local community resources and support services.
- A strong personal faith and commitment to integrating spirituality into the recovery process.
- Certification in case management or addiction counseling is a plus.
