Salary Range: $15.50 - $17.05 Hourly
Description
- Provide home visitation services to pregnant women, interconception women, and children three and under who are at risk for poor health outcomes
- Provide family centered, strength based interventions with sensitivity toward cultural differences
- Assess risks and link individuals to needed resources, provide information, guidance, and support to individuals so that maternal-child health outcomes, child development outcomes, and home environment is more safe, healthy and nurturing
- Family support systems will also be increased through referrals and individual family support plans based on assessed needs
- Provide parenting education as needed and as requested by the participant in order to enhance parent-child relationships and to reduce the likelihood of child abuse and neglect
- Provide smoking cessation services with participant and/or household members in an effort to promote healthy birth outcomes and child development
- Caseloads include face to face and phone contact as well as more intensive home visitation
- Willing to attend all service training within specific timeframes and participate in ongoing staff development
- Adhere to timeliness standards for all documentation in the Well Family System (WFS), including coding of the Initial Assessment, Family Support Plan, and all other services and referrals
Qualifications
- High school diploma or equivalent
- Experience working with or providing services to children and families
- Knowledge of normal maternal-child health, child growth/development, parent-child relationships, and smoking cessation
- Must complete Pre-Service and In-Service Training prior to providing services
- Ability to pass a level II Background clearance and drug test.
- Valid FL drivers’ license, insurance, and safe driving record.
- CBHC New Hire Orientation (NHO) completed within first two (2) months of employment.
- Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures.
- Ability to use a computer.
- Attention to detail.
- Ability to work independently and as part of a team, in collaboration with other community partners.
- Ability to manage stressful situations and display appropriate work demeanor and boundaries.

- Experience with home visitation is preferred
- Ability to perform repetitive tasks.
- Ability to sit or stand for extended periods.
- Ability to safely operate a motor vehicle in all driving conditions.
- Ability to lift up to 10 pounds
