JOB SUMMARY: |
As a critical member of interdisciplinary care teams and working in close collaboration with the Director of Diabetes and Nutrition Services, the Diabetes and Nutrition Program Navigator helps patients navigate through Diabetes and Nutrition program (“the Program”) at Chase Brexton Health Care. This position is responsible for coordinating interdisciplinary diabetic care in a timely and comprehensive manner. Responsibilities include, but are not limited to, facilitating outreach processes and scheduling, closing referral loops, communicating questions and needs to providers, pharmacists, nutritionists, and nurses, and updating patient medical records to reflect relevant outside documents such as outside imaging, consults, etc. This position is a split between Chase Brexton’s Mount Vernon, Glen Burnie, Security Square and Columbia clinic locations as well as Easton remotely. |
