JOB SUMMARY:

As a critical member of interdisciplinary care teams and working in close collaboration with the Director of Diabetes and Nutrition Services, the Diabetes and Nutrition Program Navigator helps patients navigate through Diabetes and Nutrition program (“the Program”) at Chase Brexton Health Care. This position is responsible for coordinating interdisciplinary diabetic care in a timely and comprehensive manner. Responsibilities include, but are not limited to, facilitating outreach processes and scheduling, closing referral loops, communicating questions and needs to providers, pharmacists, nutritionists, and nurses, and updating patient medical records to reflect relevant outside documents such as outside imaging, consults, etc. This position is a split between Chase Brexton’s Mount Vernon, Glen Burnie, Security Square and Columbia clinic locations as well as Easton remotely. 

 

MAJOR DUTIES AND RESPONSIBILITIES:

Planning and Organizing

  • Coordinates continuity of patient care by seamlessly coordinating the referral process to the Program. This may include, but not limited to, outreaching, scheduling diabetes and nutrition program visits, modifying schedules, coordinating communication between the Program and PCP teams, obtaining records / documents, documenting coordination efforts, triaging calls between the patient and the teams, etc.
  • Coordinates the transition process by communicating with the PCP’s team when patients graduate from the Program.

 

Communication

  • Communicates clearly, respectfully, and compassionately with patients and caregivers while delivering services and during telephonic outreach.
  • Coordinates with all members of the care team within the organization to convey pertinent patient issues, appointment information, provider schedules, and other information.
  • Actively listens to patients, caregivers, and members of the care team.

 

Workplace Computers and Equipment  

  • Effectively use EMR, care-management programs, reporting software and other applications to review and updates relevant diabetes-related information as directed by supervisor.

 

Teamwork

  • Consistently assists other members of the interdisciplinary patient care team.
  • Participates in regular meetings with clinical care teams, one-on-one meetings with supervisor and other meetings as assigned by supervisor.

 

Problem Solving and Decision Making

  • Identify barriers to care and connecting patents to appropriate supportive services or resources.

 

Checking, Examining, and Recording

  • Confirms demographic information during each patient encounter, both telephonic and in person.
  • Accurately documents medical data and patient communications.
  • Other duties as assigned

 

SKILLS AND ABILITIES:

  • Excellent customer service skills
  • Excellent interpersonal skills
  • Excellent phone etiquette. Knows how to smile over the phone 
  • Proficient in Microsoft Office Outlook, Word, and Excel
  • Ability to work with multiple healthcare-based computer applications
  • Ability to learn quickly 
  • Ability to adapt to change
  • Team player
  • Able to follow through with delegated tasks 
  • Motivational Interviewing training a plus

EDUCATION/EXPERIENCE: 

Required: High school diploma or GED

Required: One year experience working in the medical field

Proficiency in Microsoft Office suite
