What are the responsibilities and job description for the Diabetes Navigator position at InsideHigherEd?
We are seeking a Diabetes Navigator who will play a central role in providing essential guidance and support to patients with diabetes on advanced diabetes technology (continuous glucose monitors and Insulin pumps) participating in a clinical trial. As the primary point of contact, this role focuses on offering comprehensive assistance throughout the process of prescription, access and optimal utilization of advanced diabetes technology. Collaborating closely with the care team and external stakeholders, the navigator actively contributes to the optimization of glycemic control and overall positive health outcomes for patients.
Specific Duties & Responsibilities
Direct Patient Care & Support
- Initiate, build, and maintain a strong rapport with patients with diabetes on advanced diabetes technology (CGMs and Insulin pumps) enrolled in the clinical trial.
- Provide comprehensive support and guidance to patients throughout the process of initiation and progression of using advanced diabetes technology.
- Collaborate with care team members including Physicians, Nursing staff, CDCES and research coordinators to facilitate timely access and initiation of use and follow-up of advanced diabetes technology.
- Liaise between insurance companies, pharmacies, and organizational representatives to eliminate obstacles that may arise at the stakeholder level, streamlining processes, and ensuring uninterrupted access to advanced diabetes technology.
- Utilize a predesigned toolkit to troubleshoot issues arising with the process of prescription, initiating use, and optimal use of diabetes technology.
- In-person and remote (i.e., video teleconference, phone) interactions with patients at multiple clinic sites within the institution across the clinical service areas

The pre-employment physical for positions in clinical areas, laboratories, working with research subjects, or involving community contact requires documentation of immune status against Rubella (German measles), Rubeola (Measles), Mumps, Varicella (chickenpox), Hepatitis B and documentation of having received the Tdap (Tetanus, diphtheria, pertussis) vaccination. This may include documentation of having two (2) MMR vaccines; two (2) Varicella vaccines; or antibody status to these diseases from laboratory testing. Blood tests for immunities to these diseases are ordinarily included in the pre-employment physical exam except for those employees who provide results of blood tests or immunization documentation from their own health care providers. Any vaccinations required for these diseases will be given at no cost in our Occupational Health office.
