General Summary

The Independent Living Coordinator is responsible for the direct oversight and management of the Independent Living Program (ILP) at The Chase Home. The Independent Living Program contracts with the State of NH to offer a Level 1 program to young adults ages 18-21. The ILP Coordinator will provide case management, support, and guidance to prepare these young adults for their future as self-sufficient, confident, and positive contributors to society. The ILP Coordinator will work in partnership with these young adults to gain necessary life skills such as education, job placement, driver's education, money management, and mental health stability.


Essential Job Functions - Independent Living Coordinator

  • Provide ongoing outreach to DHHS offices for ILP referrals.
  • Always maintain a census of 2-4 young adults
  • Manage the ILP budget and practice appropriate fiscal spending.
  • Responsible for all intakes and discharges.
  • Assist the young adults in ILP to always maintain a clean, safe, and organized environment.
  • Ensure all data entry in Apricot (electronic case management system) is completed and delivered in a timely and efficient manner.
  • Ensure all ILP services are coordinated and integrated in an organized and consistent manner.
  • Maintain and ensure quality assurance standards are met regarding the treatment planning, record keeping, and documentation of the program.
  • Oversee the services and treatment plans of the young adults with a sense of urgency to help launch the young adults into adulthood successfully.
  • Oversee, coordinate and structure the weekly ILP groups.
  • Participate and complete all assessments, surveys, and related activities for DHHS.
  • Collaborate and coordinate with DHHS and all supporters of each young adult with the common goal of providing high quality and wrap around support.
  • Establish goals/metrics to regularly evaluate the ILP to further enhance and iprove the overall program.
  • Continuously consider and research new and relevant processses and initiatives to expand services in the best interest of the young people served
  • Continually work with treatment teams of residents in Level 2/Residential Treatment Program to identify, support, and prepare those residents who can appropriately transition to ILP/Level 1 upon their 18th birthday.
  • Participate in crisis prevention and intervention as needed.
  • Responsible for creating and actively updating all policies, procedures and handbooks related to the ILP.
  • Attain RENEW certification to provide RENEW services to each young adult.
  • Directly supervise the ILP Case Manager.
  • Participate in Administration On-Call.
  • Other duties as assigned

Qualifications

  • Bachelor's degree in human services or equivalent.
  • Minimum of 5 years experience working with at-risk young people
  • Preferred prior work experience in a youth residential treatment setting.
  • Preferred leadership experience as supervisor and/or manager.

  • Relationship with Youth - Maintains interactions with the youth; ability to form interpersonal and positive relationships; maintain appropriate boundaries; strength-based approach; utilize MANDT and TBRI intervention; participate in ILP programmatic activities as necessary; Provide RENEW services as needed
  • Member of the Management Team - Attend management meetings; participate in administration on-call; supervise staff/interns as appropriate; follow all confidentiality protocols; always maintain professionalism in position

  • Must posess the ability to handle sensitive information and maintain confidentialiy
  • Excellent customer service skills; highly proficient in Google, Word, Excel

Working Conditions / Physical Demands

  • Ability to bend, reach, move and lift 75 lbs
  • Ability to ascend and descent multiple stairways
  • Visual and hearing acuity to perform job related functions
  • Ability to work a flexible day schedule
