Position Summary :
Are you looking for a job that loves you back! ChesPenn Health Services is seeking a Community Health Worker with a dedication to serving the community. The Community Health Worker (CHW) serves as a bridge between ChesPenn and our patients and their families to help improve their health. The CHW uses his / her knowledge of the local community and the health and human services resources in the area to empower patients and their families to understand and mange their health needs.
Requirements :
- High school diploma or equivalent
- Current COVID-19 and flu vaccines required
- A valid driver's license
- Will be required to utilize your own car
