SOCIAL SERVICE DIRECTOR
Chestelm Health & Rehabilitation is looking for a full-time, Director of Social Work to join our team
The primary role of the Director of Social Work is to coordinate, supervise and facilitate the delivery of Social Work Services, as provided by facility or facility contracted staff, to all residents of the Facility, and in accordance with applicable state and federal regulations and professional standards. This position fulfills the responsibility of direction of the facility’s social work services as required by both the State of CT and Federal guidelines. Chestelm is a 76 bed 5 STAR CMS RATED skilled nursing facility. We embrace the family atmosphere with a solid team approach.
Responsibilities:
Actively participates in developing and implementing new behavioral and psycho social service programs and the presentation of such programs to physicians, family members, and residents.
Qualifications:
Bachelor’s Degree in Social Work or related human services field, required.
A minimum of one year’s experience in long term care providing direct social services, required.
Key Responsibilities:
- Responsible for implementing and documenting a plan of care related to the resident’s social/emotional needs in the electronic medical record.
- Conduct face-to-face resident interviews and assessments.
- Responsible for collaborating with the nursing, MDS and rehabilitation departments to facilitate discharge plans for residents.
- Informing and educating the staff/resident/family/responsible party of the resident’s personal and property rights, as well as his/her rights related to health services as pertaining to the resident.
- Maintains updated resources for residents and families regarding community resources and services, as well as service agencies and organizations. Maintains a record of all reported concerns, assists in resolving them and assures appropriate follow-up and documentation.
- Easily accessible to residents, family and staff. Provides timely referrals to appropriate agencies for assistance to the resident/family (i.e. conservatorship application, Power of Attorney, Insurance Notifications, Medicaid application, etc).
Requirements:
- Previous experience in a long-term care facility.
- Must be knowledgeable of admissions and discharge procedures in conjunction with all current laws, as well as be familiar with residents’ rights and documentation requirements in accordance with current regulations and/or guidelines.
- Attention to detail, good follow through skills and ability to prioritize multiple tasks.
- Must be innovative and able to find creative ways to serve the variety of resident needs and maintain the customer focus.
- Demonstrates cooperation in assisting others and ability to work as a team.
- Patience, tact, enthusiasm and a positive attitude toward the residents and their support networks.
- Ability to remain calm in stressful situations.
- Actively participates in developing and implementing new behavioral and psychosocial service programs and the presentation of such programs to physicians, family members, and residents.
- Ability to communicate with residents, staff and family members in a professional manner.
- Attend and/or organize professional or required meetings such as Family Council, Leadership, or Quality Improvement.
