Chesnut Hill of East Longmeadow , a Skilled Nursing Facility in East Longmeadow, MA, is currently seeking applicants for an experienced MDS Coordinator .
Requirements :
- Graduate of an accredited school of nursing with current LPN / RN licensure in Massachusetts
- One year of Long-term care experience
- Experience with Medicare / Medicaid reimbursement, MDS completion, clinical resources utilization, and / or case management is highly desirable
