Overview
Child and Family Connections #20, a program of Community Support Systems, is accepting applications for a full-time Early Intervention Service Coordinator. Child and Family Connections is a regional intake program for the Illinois Early Intervention System and is responsible for ensuring all qualified children under age three receive Early Intervention (EI) services. Service Coordinators support families by working collaboratively as a team to help determine eligibility for EI, support the development of the Individualized Family Service Plan (IFSP), coordinate the delivery of direct services, and assist families with the development and implementation of a transition plan when their children turn three. Service Coordinators work with an assigned caseload throughout an eleven county service area - Effingham, Christian, Bond, Clay, Crawford, Fayette, Jasper, Lawrence, Macoupin, Montgomery, and Richland. Job duties are primarily conducted virtually.
Primary responsibilities include, but are not limited to:
- Conduct intake process including collection of pertinent family documentation, complete initial and annual eligibility determination process, and facilitate all Individualized Family Service Plan (IFSP) meetings in accordance with the Illinois Department of Human Services (IDHS) timelines and procedures.
- Coordinate the completion of evaluations and assessments necessary for the development and review of the IFSP and assist family in monitoring progress.
- Maintain comprehensive permanent records, including correspondence and case notes.
- Navigate online data management systems.
- Coordinate the distribution of IFSPs and assessments to multiple Early Intervention providers, school districts, and health care providers, as appropriate.
- Inform families of their rights and the availability of services and maintain monthly communication.
Requirements include:
- Bachelor degree or higher in Human Services, Social Work, Early Childhood, Special Education, or related field.
- Requires highly organized and self-motivated individuals with excellent communication skills.
- Experience with case management (preferred).
- Knowledge about child development.
- Experience with virtual modalities and platforms.
- Reliable transportation, valid driver's license, and current insurance.
- Successful completion of IDHS Service Coordinator training requirements in order to receive an Early Intervention Service Coordinator credential (and maintain said credential).
- Successful background check and acceptable driving record.
