Position Summary
The Certified Peer Support (CPS) Program Coordinator is responsible for providing supervision and oversight to CPS staff and managing the coordination of new clients being referred to the Certified Peer Support Program while maintaining a CPS caseload. They will engage in marketing efforts and manage and maintain the expected client and staff census. This position will include travel within Bucks, Montgomery, and Delaware Counties.
Required Qualifications
- Be credentialed as a Certified Peer Specialist through the PA Certification Board.
- Complete the Department-approved CPS supervisory training within six months of hire
- Bachelor’s degree in human services, social work, psychology, or a related field; with two or more years of mental health direct service experience
- Associate’s degree in human services, social work, psychology, or a related field; with three or more years of mental health direct service experience
- High school diploma or GED; with four or more years of mental health direct service experience
- Have prior experience and knowledge of children’s mental health serving systems.
- Exhibit stability and competency in personal wellness and recovery and use of coping skills.
- Be able to reflect on the culture and understand the values of the Transition Age Youth population.
Responsibilities:
- Maintain a CPS caseload, performing all duties specific to a CPS position, and cover unstaffed CPS cases
- Responsible for monitoring the progress and performance of CPS staff and their caseloads.
- Maintain program Census List
- Establish and ensure a strengths-based, client-driven culture of service provision
- Lead recruitment, onboarding, and training of new staff while driving initiatives to enhance staff retention and foster a positive, high-morale work environment
- Offer feedback and coaching to support the achievement of daily program expectations
- Manage incoming referrals, enrollment, and client tracking, and determine client eligibility for program admission
- Coordinate intakes and assign/manage staff caseloads
- Review and ensure the timely and accurate submission of all staff documentation
- Work with staff to stay engaged with clients and identify appropriate resources
- Communicate with clients/families who may be disengaged from services and problem-solve engagement issues
- Assess the quality of client care/satisfaction and resolve disputes
- Support proper discharge planning
- Deliver direct field support/supervision to staff and support staff with case-specific needs
- Ensure elements of service delivery are followed according to program regulations
- Monitor chart auditing processes and ensure timely documentation submissions
- Provide additional support and supervision for intensive cases – attend ISPTs, support crises, field complaints, etc.
- Network and coordinate outreach for referrals, and assure a steady flow of referrals to maintain client census
- Maintain a working relationship with local school districts and mental health providers, enhancing access to care
- Participate in resource fairs and community engagement opportunities
- Attend stakeholder meetings, as needed
- Engage in supervision with CPS Director
- Transport youth to various community locations
- Perform other duties, when required or assigned
Work Environment:
- Manage a varied work schedule of 40 hours per week, Monday-Friday between 8:30 AM - 8:30 PM, including at least 3 evenings per week.
- Work in standard office environments and community settings, including client homes, schools, courts, doctors’ offices, and hospitals.
- Substantial (50%) local travel in your own vehicle to off-site locations as required by caseload or training requirements, within Bucks, Montgomery, and Delaware Counties.
