What are the responsibilities and job description for the Family Assessment Worker/Family Support Specialist position at Child & Family Resources?

GENERAL DESCRIPTION:
The Family Assessment/Family Support Specialist is responsible for initial identification, referral and coordination activities for high-risk families of newborns and for providing home-based prevention services focusing on supporting the parent child relationship, attachment, and family enhancement. This is done through child development education, play, goal setting, assessments, and coordination of services/activities for overburdened families. This position reports directly to the HF Supervisor.

SUMMARY OF ESSENTIAL JOB FUNCTIONS:
  • Provide direct support to postpartum women identified as high risk.
  • Conduct the screening process for initial identification in various hospitals in the community.
  • Receive daily referrals from hospital personnel as well as other agencies.
  • Administer the Family Stress Checklist on all referrals to determine risk level and need for early intervention services.
  • Obtain permission for Family Support Staff to visit the home.
  • Ensure referral to the Family Support Specialist for continued follow-up and support services.
  • Maintain client records in accordance with program and funding source requirements.
  • Meets weekly with Supervisor to discuss and review each referral.
  • Attends and participates in all scheduled Healthy Families conferences, seminars, in-service training and parent events.
  • Make CPS referrals as appropriate.
  • Conducts home visits virtually or in-person and assesses family needs and provide case management services when necessary, while promoting self-sufficiency.
  • Utilizes child development curriculum with each family and promote positive parent-child relationships.
  • Utilizes floor time to support play within the parent and child relationship.
  • Utilizes appropriate limit setting and boundaries.
  • Acts as a positive role model.
  • Maintains necessary assessments and documentation in accordance with agency and funding source requirements.
  • Participates in and comes prepared for weekly supervision with Program Supervisor.
  • Coordinates with other community services.
  • Participates in agency Continuous Quality Improvement activities.
  • Perform other related duties as assigned.

REQUIREMENTS:
  • Bachelor’s Degree in Social Work or related behavioral health field required and two years of experience in related field preferred.
  • Experience providing services to at-risk children and families.
  • Ability to establish trusting relationships and interact effectively with families.
  • Knowledge in family life skills.
  • Knowledge of infant/child development (willingness to obtain an Infant Mental Health Endorsement).
  • Knowledge of and sensitivity to individual/cultural differences and experience working with culturally diverse populations.
  • Openness to reflective practice (capacity for introspection and recognition of value of supervisory support).
  • Ability to communicate effectively both orally and in writing.
  • Ability to work effectively on a family support team.
  • Flexibility in scheduling (able to work limited nights and weekends).
  • Bilingual Spanish strongly preferred. if not bilingual, able to describe program in Spanish (script will be provided).
  • Location: must be able to provide direct service to the designated client base in the Colorado City area.
  • Maintain a functional and safe home working space with adequate internet connection for the job.
  • Comply with all agency and program policies and procedures including confidentiality, employee conduct, computer usage and dress code.
  • Meet program/department’s performance and productivity standards.
  • Attend required agency and program/department meetings.
  • Adhere to the schedule agreed upon with the supervisor.
  • Follow and model CFR’s core values.
  • Participate in PQI initiatives.
  • Certification in CPR/First Aid and/or ability to obtain such.
  • Must be eligible and able to obtain a Level One fingerprint card.
  • Must have a valid Arizona driver’s license, and vehicle and meet agency liability insurance requirements.

ABILITY REQUIREMENTS:
  • Sitting for extended periods of time daily; standing and walking short distances.
  • Must be able to climb stairs and move through a facility and a home.
  • The position includes interaction with children that could include picking up a child as heavy as 40 pounds.
  • Moderate twisting, bending, stooping and lifting up to 40 pounds in the performance of assigned duties.
  • Ability to use a variety of office equipment and machines as referenced.
  • Normal manual dexterity and eye-hand coordination required.
  • Repeated hand-wrist movement required.
  • The nature of the position involves a fast-paced working environment with multitasking of many different duties and tasks. General daily priorities may change at a moment’s notice and the position requires quick response time and flexibility.
