OhioRISE Care Coordinator
Our OhioRISE Care Coordinator will receive a competitive sign on bonus!
Location : We are hiring for a 6-county region of Hamilton, Clermont, Brown, Adams, Scioto and Lawrence counties.
Degree and Experience Requirements
- High School Diploma / GED
3 years of case management experience
Bachelor’s Degree in Social Work, Psychology, Criminal Justice, or Human Services2 years of case management experience
Master’s Degree in Social Work, Counseling, Psychology, Criminal Justice, or Human Services1 year of experience
Additional Experience :Experience must include working in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a human services field, providing community-based services to youth and their caregivers.
Primary Job Responsibilities
OhioRISE Initiative
Participate in Ohio Medicaid’s program designed to deliver high-level care coordination and tailored services for youth and families with complex care needs.Care Coordination
Serve as the primary point of contact for assigned OhioRISE youth.Provide comprehensive care coordination addressing youth and family service needs.Collaborate with professionals across education, behavioral health, juvenile justice, developmental disabilities, hospitals, and family councils.Service Planning
Deliver structured service planning and care coordination through high fidelity wraparound practices.Execute care coordination activities in line with OhioRISE guidelines.Complete comprehensive CANS assessments along with ongoing evaluations.Convene and facilitate child and family teams for the development and review of care plans.Crisis and Transition Management
Develop crisis safety plans.Provide referrals and linkages to appropriate services and supports.Facilitate discharge and transition activities for youth across various settings.Advocacy and Empowerment
Advocate for the best interests of clients and their families.Empower clients and families to identify their own needs and resources.Documentation and Compliance
Complete and maintain all required documentation.Maintain professional licensure (if applicable) and demonstrate proficiency with Electronic Health Records.Complete required OhioRISE / CARF / Child Focus training.Participate actively in staff development and team meetings.Professional Conduct
Uphold a professional and ethical standard in all interactions.Work respectfully with culturally diverse populations.Exhibit strong organizational and time management skills.Maintain a Care Coordinator to ICC youth roster ratio of 1 : 10 or a 1 : 25 ratio for MCC youth.Benefits
