What are the responsibilities and job description for the Community Link - Housing Caseworker position at Children Family Services Center?
JOB PURPOSE
Under the guidance of a Program Manager, provide casework services which include referrals to in-house and community resources. Honor the values and ethics of Community Link and the social work profession while providing prejudice-free and diversity-sensitive services to customers. Provide planning, oversight and support to individuals and families through assistance with obtaining and sustaining safe, decent and affordable housing.
RESPONSIBILITIES / ACCOUNTABILITIES
Casework Services
- Conduct thorough assessments of all new customers and assist during periods of crisis and/or transitions by utilizing appropriate social work methods.
- Participate in Intake/ Traveler's Aid services rotation, providing telephone and face-to-face information about Community Link programs and community resources.
- Coordinate customer referrals, as appropriate, to community resources and to internal resources for financial coaching program and homeownership counseling.
- In partnership with customers, develop, review and amend efficient and effective case plans.
Legal Aspects
- Maintain confidentiality of all program documents and records.
- Adhere to NASW Code of Ethics.
- Maintain participant and CoC Program documentation, including all financial and programmatic records, for a period of at least five years in accordance with 24 CFR 578.103 (c).
Administration
- Issue commitment letters to all CoC customers and landlords regarding program status.
- Attend and actively participate in interagency staff and multidisciplinary meetings.
- Compile and submit reports to Program Manager as assigned.
Database/ File Maintenance
- Ensure customers sign a consent form regarding the database.
- Update and maintain pertinent information.
- Input data in a timely manner (24 hours).
- Enter CoC Program information into the Homeless Management Information System (HMIS).
- Accurately record case notes, update plans and recertify as required.
- Maintain confidentiality of all program records and documents.
EDUCATION and/or EXPERIENCE
- Bachelor's Degree Required, Master's degree of Social Work preferred.
- Valid Driver's License.
- Two-Three years' experience in social services assessment and service delivery.
- Experience based understanding of protocols related to real estate taxes, utilities management, and similar factors related to home occupancy
- Effective familiarity with the Residential Real Estate sector and related codes
- Preferences: Bilingual in Spanish Preferred.
