The Case Navigator is responsible for providing RSS contract services in the office, community, and home as needed to meet program goals which include: (1) support a greater number of Kinship and NREFM caregivers both formally placed through DCFS and informal arrangements, (2) retain caregivers to become resource families for future children, and (3) when reunification is not possible, increase guardianship/adoption with family. The case navigator will work directly with referred families to assess their needs and link to services provided both by Children's Bureau and other community providers.Duties and Responsibilities
- Attend and complete Relative Support Services (RSS) training.
- Contact and initiate services to clients within 48 hours of referral.
- Assist relatives access Relative Caregiver Emergency Needs funds.
- Provide advocacy including but not limited to assistance with child education, adoption, and guardianship issues.
- Assistance with eligibility for governmental benefits
- Development of a network of agencies to provide a variety of services to kinship caregivers.
- Complete necessary documentation as required.
- Participate in outreach campaigns including at minimum two (2) annual events for caregivers, attendance at network meetings, and canvasing the area to provide material about program.
General
- Maintains confidentiality in matters of agency operations, personnel, and clients.
- Maintains positive communications with all agency staff, clients and visitors.
- Exercises good judgment in performance of duties and responsibilities.
- Supports existing agency policies, principles and mission.
- Other related duties as assigned.
Job Qualifications:
- Thirty-two (32) units from an accredited University, College or Community College, with at least 16 units in Social Work, Social Sciences, Humanities, Public Administration or Public Health OR
- Two (2) years minimum experience in the field of social work, behavioral science or social science performing specialized kinship support services in a social services agency OR
- Former kinship caregivers previously approved by DCFS or Probation Department for kinships/relative placements, with no open case with DCFS or Probation Department, with approval from the County.
- Have flexibility in schedule to work in the office on Saturdays as assigned.
- Ability to write clear, concise, efficient reports and meet deadlines for submission.
- Must have reliable transportation, valid driver license, current vehicle registration and automobile insurance.
- Provide a cell phone for work purposes (reimbursement based on agency's scale provided)
- Proficient in MS Office programs, especially Word and Outlook (email) and comfortable with technology
- Bilingual Spanish
