Job Summary
Under the supervision of the Director of Relative Home Assessment Services and Relative Support Services (RHAS/RSS), the RHAS/RSS Lead Case Navigator will be responsible for training new staff, reviewing and overseeing subcontracts, tracking RSS outreach efforts, and coordinating and planning RSS events. Additionally, they will manage an RSS caseload and lead training sessions for resource families. The RHAS/RSS Lead Case Navigator closely collaborates with the Director of RHAS/RSS regarding the needs of clients and staff.
Essential Functions
- Assist RHAS/RSS Director in maintaining programmatic needs.
- Assist in training any new staff that enter the program
- Lead the planning of the RSS Appreciation Event and RSS Resource Fair
- Attend any meetings as assigned.
- Monitor and review subcontracts and monthly services rendered.
- Weekly monitor outreach RSS cases, creating initial case logs, verifying demographic information and providing status updates.
- As an RFA Trainer, provide 12 hours of training to incoming and current resource families
- As an RFA Trainer, provide 8 hours of recertification training for resource families within the first year of the approval date.
- Attend staff trainings and in-service trainings.
- Maintain RSS caseload by advocating and providing resources for caregivers and referring them to resources based on various needs including finding food banks, mental health services, utility assistance, rental assistance, etc.
- Sensitive and responsive to the cultural differences among the agency’s client population
- Works to promote and support justice, equity, diversity, and inclusion at all levels in the organization.
- Works to promote a trauma-informed work environment.
- Actively participate in quality improvement projects
The RHAS/RSS Lead Case Navigator will take on special projects or additional duties as assigned by the Director of RHAS/RSS.
Qualifications
Competency Requirements
-Ability to work as a member of multidisciplinary team
-Ability to read, write, speak, and understand English
-Must have excellent oral and written communication skills
-Must have excellent organizational skills
-Ability to remain calm and professional in challenging or stressful situations
-Ability to maintain appropriate and professional boundaries with clients, families and staff
-Bilingual (Spanish) preferred
Education and Experience
-Bachelors' degree in Social Work or Psychology- preferred
-Minimum 2 years’ experience providing direct behavioral support to children and/or adolescents required
-Previous experience collaborating with referring agencies (Department of Mental Health, Department of Probation, Department of Children and Family services, Los Angeles Unified School District) preferred
Certifications/Licenses
-Valid California driver’s license
Additional Requirements
-Tuberculosis clearance
-Department of Justice/Fingerprint clearance
-Satisfactory completion of health screening
-Proof of legal eligibility to work in the United States
-Satisfactory driving record
-Must meet Child Abuse indexing requirements
-Current automobile insurance
Physical Demands
While performing the duties of a RHAS/RSS Lead Case Navigator the employee is often required to drive, sit, walk, and stand. The employee is occasionally required bend, kneel, and use their fingers and hands to manipulate a keyboard or writing instrument
Work Environment
The work environment is indoor and outdoor. The noise level varies from quiet to loud
