What are the responsibilities and job description for the Outreach Coordinator position at Children's Home Network?
JOB SUMMARY: Develops referral sources and partnerships through outreach within the community and with neighborhood leaders. This position promotes partnerships on local events, identifying key partners and developing new relationships in high need communities.
ESSENTIAL FUNCTIONS:
- Actively meets with community programs, leaders, faith based organizations to develop new relationships and partnerships.
- Works with Program Manager to identify community based relationships.
- Completes timely documentation on outreach efforts and partnerships.
- Ensures Program Manager is well informed about outreach efforts.
- Collaborates with neighborhood centers, faith based organizations and community stakeholders to identify target population.
- Coordinates interagency communication and activities.
- Actively monitors referral sources in quality improvement measures to help ensure that
contracted service levels are being met.
- Acts as resource to neighborhood centers, faith based organizations, community stakeholders and collaborative partners.
- Attends training, participates in internal/external committees, and attends all internal
meetings as directed.
- Generates a minimum of 8 referrals to the program on a monthly basis.
- Works within the philosophy and function of The Children’s Home Network.
- Performs other duties as assigned.

MINIMUM QUALIFICATION REQUIREMENTS:
Education & Experience:
High School diploma or equivalent and two (2) years experience in Social Services environment
or related field. Previous experience conducting outreach and generating program referrals and knowledge of Pinellas County services and resources preferred.
Licenses & Certifications:
- Must possess and maintain a valid Florida driver’s license with no record of criminal
driving offense of license suspension.
- Must possess means of providing job related transportation and show proof of required
insurance. Must be insurable under CHI’s current auto insurance policy.
- Must be able to work flexible hours, including evenings and weekends.
- Must successfully complete and maintain FDLE clearance, Federal background and
state criminal background check and sexual predator screening.
PRIMARY CLIENTS SERVED:
Families identified as Caregivers of children between the ages of 0-14 who have special needs that are self-referred or through community referrals.
