We are seeking a skilled Foster Care Case Planner to join our team at Children's Home of Jefferson County. The ideal candidate will have experience working with at-risk youth and their families, with a focus on Strength-Based and Individualized Care Approach.
Responsibilities
- Complete thorough assessments of family dynamics and create personalized treatment plans.
- Negotiate and coordinate visitations between youth and resource families.
- Communicate regularly with service providers and schools to ensure cohesive support systems.
- Maintain accurate documentation of all case work activities and case files in compliance with OCFS policies.
Skills and Qualifications
- Bachelor's degree in social work or related human service discipline (preferred).
- At least two years' experience working with at-risk youth and families.
- Strong understanding of Child Welfare laws and regulations.
