Become a vital member of the NEW Home-Based Crisis Intervention (HBCI) Team in Broome County at the Children’s Home of Wyoming Conference. Make a meaningful impact on youth aged 5-20 with behavioral health needs through short-term therapeutic interventions that support families in their own homes and communities. This position requires travel within county.
Responsibilities

Provide supportive clinical services to 2-3 families at a time, conducting regular to daily meetings with each child/youth and family.

Deliver crisis intervention, prevention, and resolution strategies.

Engage in frequent outreach to clients and their families.

Conduct strength-based assessments to understand clients’ social determinants of health, mental well-being, resilience to trauma, potential risks, and substance use factors.

Create personalized service plans tailored to individual family needs.

Design and implement effective service delivery strategies.

Collaborate with partner service providers to coordinate comprehensive treatment plans.
- Maintain thorough case management records to ensure effective service delivery.
Requirements
Education
-
Bachelors in human services, psychology, social work or other related fields with at least 2 years experience in case management, community based mental health, and other related responsibilities

MSW preferred

Masters in Psychology, Social-Welfare, Human Services or other related fields preferred

Licensed mental health provider or unlicensed Masters preferred
- Bachelor's - 2 years experience in case management, community based mental health, and other related responsibilities
- Must have a VALID driver's license and ability to maintain insurance throughout employment required
