Job Purpose
We are seeking a dynamic, detail-oriented Outreach and Enrollment Coordinator to join our team and play a key role in promoting and educating the community about the PACE (Program of All-Inclusive Care for the Elderly) program. This position will be responsible for attending outreach events, conducting program tours for walk-ins, assisting with data entry for enrollments and new prospects, and supporting efforts to increase awareness of the PACE program.
Duties and Responsibilities
Community Outreach & Education :
- Represent the PACE program at local community events and informational sessions.
- Educate and inform potential participants and their families about the PACE program and its benefits.
- Promote the PACE program through various community engagement opportunities, both in-person and virtually.
Enrollment Support :
Assist with data entry and management for all enrollment processes, ensuring accuracy and completeness of participant information.Assist with processing and tracking new enrollments.Collaborate with the enrollment team to ensure timely and efficient handling of all enrollments.Conduct Program Tours :
Provide guided tours for walk-in inquiries and prospective participants, showcasing the facilities and explaining the benefits and services offered by the PACE program.Address questions and concerns during the tour, offering detailed explanations and support to ensure prospective participants feel well-informed and comfortable.Attend Enrollment Conferences :
Conduct enrollment conference for the PACE prospective participants and ensure all enrollments are completed on time.Stay updated on enrollment procedures, best practices, and any program updates discussed at these events.Data Entry for New Prospects :
Assist in the timely and accurate entry of information for new program prospects into the database.Ensure that all prospect data is recorded correctly to maintain the integrity and quality of the enrollment pipeline.Other duties as assignedSkills and Knowledge :
High school diploma or equivalent; a college degree in healthcare, social services, or a related field is preferred.Previous experience in community outreach, enrollment coordination, or healthcare services is preferred.Strong organizational skills and attention to detail.Excellent verbal and written communication skills.Ability to engage with diverse community members and potential participants.Familiarity with data entry software or CRM systems is a plus.Ability to work independently and as part of a team.Desired Personal Attributes :

Must be able to work required schedule.Requires physical strength to perform essential functions of the job.Requires use of personal vehicle.Requires valid driver’s license.Requires some evening and weekend for community events or outreach activitiesRequires proof of automobile insurance coverage at the following minimum amounts in order to be reimbursed for mileage : $100,000 / $300,000 personal liability and $100,000 property damage.May require use of personal cell phone for business purposes (may be eligible for stipend)Physical Demands
Must be able to remain in a stationary position 50% of the time.Ability to occasionally move about inside the office to access file cabinets, office machinery, etc.Able to operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.Able to constantly position yourself to maintain files in file cabinets such as reaching with hands and arms, kneeling, crouching, etc.The ability to communicate, detect, converse with, discern, convey, express oneself, and exchange information is crucial for this role.Compliance with the Americans with Disabilities Act (ADA)
