ORGANIZATION DESCRIPTION
Founded in 1965, the Chinese-American Planning Council, Inc. (CPC) aims to promote the social and economic empowerment of Chinese American, immigrant, and low-income communities. Today, CPC is the nation's largest Asian American social services agency and offers more than 50 programs at over 35 sites throughout New York City, including early childhood education, school-age care, youth services, workforce development, community services, and senior services. PROGRAM SUMMARY
CPC Open Door Older Adult Center provides individuals over 60 years old with an array of services designed to assist seniors emotionally, mentally, and physically. The program provides educational, recreational, and cultural activities/workshops that promote an inclusive, active, and healthy lifestyle for seniors.
JOB SUMMARY
Reporting to the Program Director and Deputy Director, the full-time Case Manager will be providing older adult services including, information referral, benefits support, case assistance, and providing support to the Program Director to ensure high quality services that respects the dignity of all the community members.
ESSENTIAL RESPONSIBILITIES
- Provide information to seniors and/or their representatives to enable them to locate and obtain needed services, benefits, entitlements, and other resources.
- Refer community members to appropriate agencies/organizations.
- Provide case assistance services by assisting older adults in defining concerns, needs and capacities; providing direction or guidance relative to identified needs, and assisting with referral and linkages to appropriated services and opportunities.
- Follow-up to determine whether services/benefits has been, or is being, provided, after a referral has been initiated.
- Develop and foster strong relationships with consultants, community partners, and government agencies.
- Maintain and continually update a directory of supportive services and agencies available to the local community
- Work alongside the Deputy Director to ensure that the center is in compliance with policies and procedures
- Maintain record and input all forms and reports as requested by program guidelines.
- Assist with new member enrollment.
- Assist with group activities and special events.
- Advocate and provide outreach for older adult services.
- Other duties as assigned by the Program Director and Deputy Director
QUALIFICATIONS
Education and Experience:
- Bachelor’s degree in social work or related field; master’s degree preferred, but not required
- 2-3 years of case management experience with the older adult population in a non-profit setting preferred
- Some supervisory experience preferred
Skills and Competencies:
- Excellent communication, organizational and interpersonal skills; knowledge of older adult and immigrant issues helpful.
- Knowledge and skills using Outlook, Word and Excel.
- Fluent in English and Mandarin and/or Cantonese is a must.
- Proven coordination skills to connect individuals/families with relevant services.
- Strong work ethic and demonstrated passion for senior services.
COMPENSATION & BENEFITS OVERVIEW
- $25.68 - $25.68 per hour; 35 hours per week
