ORGANIZATION DESCRIPTION

 

Founded in 1965, the Chinese-American Planning Council, Inc. (CPC) aims to promote the social and economic empowerment of Chinese American, immigrant, and low-income communities. Today, CPC is the nation's largest Asian American social services agency and offers more than 50 programs at over 35 sites throughout New York City, including early childhood education, school-age care, youth services, workforce development, community services, and senior services. CPC strives to be the premier social service and leadership development organization. In 2017, CPC launched Advancing Our CommUNITY, its organization-wide strategy to expand services to address persistent needs and emerging trends and to improve leadership skills among staff and community members. In 2022, our core programs and emergency services reached 200,000 people living in all five boroughs and all 51 city council districts. In the summer of 2022, CPC was recognized as the number one Chinese American organization in New York City to recommend to a Chinese immigrant looking for in-language and culturally competent resources according to a recent survey. PROGRAM SUMMARY

 

Beyond Bridges Community Health Initiative is a collaborative effort between New York University (NYU) Langone Health and other Sunset Park community organizations to build a network for collaboration and referrals for better health outcomes in the communities we serve. CPC’s role as a partner is to identify and address the social determinants of health, assess the effectiveness of services rendered, and counter the issues in our communities that prevent healthy lifestyles.

 

JOB SUMMARY

 

Reporting to the Beyond Bridges Supervisor and Director of Brooklyn Community Services (BCS), the Health Linkage Navigator will focus on community member intake, community member relationship management (CRM) and Beyond Bridges referral networks. The Navigator will provide warm hand-off referrals to internal and/or external networks for community members, and survey community members for needs assessment including for health needs. The Navigator will receive referrals and may provide direct benefits access support. The Navigator will also assist with other CPC Brooklyn Community Services programs, which may include outreach, workshops, record-keeping, and data analysis.

 

ESSENTIAL RESPONSIBILITIES

 

  • Utilize Salesforce, Findhelp, UniteUs for intake, data, and referrals.
  • Coordinate referrals to and from NYU Langone Health, community-based organizations, and other health and community partners.
  • Keep a comprehensive list of referral partners and services.
  • Provide direct social services to community members, when applicable.
  • Support and organize health related initiatives and events such as surveys, screenings, health education, etc.
  • Serve as an advocate for community members and advocate for access to better health outcomes through policy and community actions.
  • Provide follow-up to assess services rendered and if additional services/referrals are needed.
  • Assist community members with language and/or benefits support as needed.
  • Assist director in organizing educational community workshops.
  • Other duties may be assigned by the supervisor and director.


QUALIFICATIONS

 

Education and Experience:

  • Minimum high school diploma or equivalent
  • Bachelor’s degree in public health, social sciences or related field preferred.
  • Experience working in and with immigrant communities preferred.
  • Experience with Salesforce, FindHelp, and UnitedUs preferred.

 

Skills and Competencies:

  • Must be bilingual in English and Mandarin and/or Cantonese.
  • Computer literacy skills
  • Able to manage multiple priorities at once to ensure core components are met on a quarterly basis.
  • Attention to detail and ability to work independently and productively.
  • Effectively works with internal staff and outside community partners, able to develop strong ties with the community, and demonstrate ability to work efficiently in a team.

 

COMPENSATION & BENEFITS OVERVIEW

 

  • $23.91 - $24.73 per hour; 35 hours per week with occasional evenings and weekends as needed
