Minimum Position Qualifications:
- College degree or equivalent knowledge and experience.
- Knowledge of behavioral health care treatment strongly preferred with demonstrated competency including word processing ability and Microsoft Office products.
- Knowledge of and experience with electronic health records systems strongly preferred.
- Above-average organizational and time management skills, with great attention to detail.
- Strong verbal and written communication skills.
- Proven ability to interact with clients, families, staff and stakeholders in a professional manner.
- Must have a current driver’s license with insurance.
- Demonstrate proven sensitivity to the various cultural and socioeconomic characteristics of CHNK Behavioral Health clients and staff, including a commitment to use of the Sanctuary Model and to the philosophy of All Children, All Families framework of cultural competencies.
Position Summary:
This position has a primary function of providing support to, and increasing efficiencies related to, the intensive services programming at CHNK Behavioral Health. This includes a primary responsibility of cofacilitating intensive outpatient therapy groups at CHNK Behavioral Health’s Outpatient location, as well as groups that are embedded in community locations.
Additional responsibilities include assistance with data entry, data tracking and monitoring and coordinating admissions and discharges for the intensive services programs. The position will also be responsible for coordinating with relevant agency personnel to enhance outcomes data and tracking.
Essential Job Functions
- Cofacilitate intensive outpatient groups in the outpatient and community-based settings
- Provides back up to Program Assistant with coordination and distribution of information related to community-based intakes; serves as back-up resource for intake process completion and phone.
- Establish and maintains community-based clinical files in the electronic health record, through monitoring compliance with case record review competency check points and monitors signatures.
- Coordinates admissions and discharges with relevant parties including parents, school districts, referrals partners and clients
- Engage in activities to gather outcomes data and other quality assurance metrics including follow up phones calls after discharge
- Ensures quality collaboration and customer service in contact with stakeholders
- Utilizes the Sanctuary based tools such as the S.E.L.F. framework in problem solving and interactions with staff, clients, stakeholders, and vendors and shows an understanding and adherence to the Seven Commitments, practicing open communication, non-violence, democracy, social learning, social responsibility emotional intelligence, and a commitment to growth and change.
