The Intake/Staffer at The Doctors Choice Home Health is responsible for managing the intake process for new clients and supporting staffing needs. This role involves coordinating patient admissions, assisting with caregiver scheduling, and ensuring compliance with insurance and regulatory requirements. The Intake/Staffer is key to creating a positive initial impression for clients and maintaining efficient staffing operations to ensure high-quality home care services.
There is also arequirementto be on-call for this position. It is done on weekends and works as a rotation 1-2x a month. A specific weekend would be designated to you, duties include answering calls on Saturday or Sunday 8 AM- 5 PM (virtually).
Essential Job Functions/Responsibilities:
- Patient/Client Coordination:
- Coordinate services for patients transitioning from hospital or related facilities to home health care.
- Support the development and implementation of patient care plans post-discharge, ensuring alignment with physician directives and agency requirements.
- Collaborate with hospital social services and discharge planning teams to facilitate home health services.
- Compliance and Authorization:
- Ensure adherence to regulatory requirements related to admissions and referrals.
- Manage the admission process with referral sources and the sales team.
- Obtain necessary authorizations for home health services from insurance providers.
- Staffing and Scheduling:
- Assign caregivers to patients based on care plans and service requests.
- Provide caregivers with accurate instructions and information regarding patient needs.
- Report scheduling changes to the Director of Intake & Staffing and notify patients of any modifications to their care schedule.
- Assist in monitoring staff performance, including dependability and responsiveness.
- Documentation and Reporting:
- Maintain accurate and up-to-date records of patient services, staffing, and scheduling.
- Complete and manage documentation, using both paper and electronic systems.
- Monitor overtime and mileage records, and address emergency scheduling needs as required.
- Communication and Support:
- Provide timely notifications of schedules and changes to employees, patients, contract agencies, and supervisors.
- Address and resolve issues related to patient care and staff scheduling.
- Adhere to HIPAA regulations to ensure patient confidentiality and privacy.
- Additional Duties:
- Support patient services with home care resources as needed.
- Ensure cases are filled within established timeframes and follow up on any scheduling or staffing issues.
Position Qualifications:
- High school diploma or GED required; Bachelors degree preferred or equivalent work experience.
- Preferred knowledge of Medicare rules and regulations, including JCAHO standards
- At least one year of experience in home health, skilled nursing facilities (SNF), or a healthcare setting.
- Familiarity with medical terminology.
- Superior multitasking and organizational abilities.
- Ability to quickly retain information and relay information to the team.
