Title: Program Director
Reports To: Vice President of Health Home Plus & OMH Services
FLSA: Exempt
Status: Full-time
Supervisory Responsibility: Team of Client Care Supervisors, Administrative Assistants, Volunteers, Interns, and other related roles.
Purpose of the Role:
The Program Director will provide the supervision to the Health Home Care Management Program, provide direct supervision to program staff to ensure the highest quality care management services are delivered in the most efficient and effective manner, represent the program at external meetings with the lead Health Home, governmental agencies and community resources to ensure the Health Home Program adopts operational processes to ensure optimal programmatic and financial functioning.
Essential Functions:
· Developing internal and external relationships that result in program growth and enrollment.
· Ensuring administrative tasks, policies and procedures are implemented and adhered to by program staff.
· Review of policies and procedures and conducting periodic reviews, updates as needed to ensure compliance with the Health Home and governmental regulations, best practices are met.
· Monitor reports on staff documentation including consents, intakes, baseline screenings, functional assessments, plans of care, progress notes and case closures to ensure they are following program policy and procedure.
· Supervising Health Home Supervisors and other program staff to ensure the delivery of high quality and efficient care management services.
· Ensure ongoing programmatic excellence.
· Build long-term, cross-sector relationships that help sustain or expand impacts of CHOICE of NY’s initiatives.
· Lead, mentor, and cultivate a high-performing team, fostering an environment of continual growth and professional development.
· Completing monthly, annual, or other periodic requests for narrative or quantitative data reports for funding sources or internal departments.
· Meeting with consumers individually and facilitate high risk case conferences as needed.
· Identifying trends in staff gaps of knowledge and make recommendations to department on how to best address.
· Overseeing and leading the efficient administration of the billing process with close detail to accuracy, quality service, and adherence to regulatory policies.
· Conduct monthly meetings with program directors to collate and assess operational challenges, collaborate on strategic problem-solving, and oversee the implementation of effective solutions across all programs.
· Collaborate closely with the Director of Data Operations and the Compliance Department to continuously enhance monthly reports, ensuring ongoing improvement in accuracy, relevance, and compliance with regulatory standards.
· Special projects and other duties as assigned.
Qualifications for this Role:
· Bachelor’s degree in social work, human services, psychology, sociology, or any related concentration preferred.
· 3 years of experience in operating or supervising social services programs, preferably in care management.
· Strong familiarity with care coordination/care management. CMAs and Health Home preferred.
· Ability to deal with sensitive issues with diplomacy and discretion.
· Unwavering commitment to quality programs and excellence in organizational and project management to achieve strategic objectives.
· Integrity, positive attitude, mission-driven, and self-directed with demonstrated passion for CHOICE of NYs mission and commitment to working collaboratively with a management team of senior professionals.
· Able to handle a variety of constituencies, manage multiple tasks simultaneously and thrive in a complex environment with multiple priorities.

· Ability to maintain a professional demeanor under pressure and operate with a keen sense of urgency and commitment to quality.
· Computer proficiency and technical aptitude; familiarity with Microsoft Office suite a plus.
Work Schedule: Monday – Friday - 35 hours per week Remote- office hybrid weekly.
Work Location: An in-office role with possibility for hybrid schedule at some point in the future.
Salary range: $55,000 - $75,000
