Join Choptank Community Health’s growing integrated behavioral health program and work as part of a multidisciplinary team, of therapists, medical and dental providers, focused on whole patient wellness. Full-time, part-time, hybrid, and flexible schedules are available. Choptank Health is a Federally qualified Health Care facility and participates with the National Health Service Corp loan repayment program. Qualified clinicians can be awarded up to 100,000 dollars.
Job Summary: Under the direction of the Director of Behavioral Health, and as a part of a multidisciplinary team, the Behavioral Health Clinician will provide direct services including individual (children and adults), couple, family, and group therapeutic services to patients.

Required Skills/Abilities:
- Working knowledge of MD Mental Health policies and procedures.
- Knowledge and experience with evidence-based practices, including trauma informed care framework.
- Strong clerical/administrative skills including personal computer skills (Microsoft Office software).
- CPR Certification
- Valid MD driver’s license
Education and Experience:
- Must have MD Licensed Clinician (LCSW-C, LCPC, LMFT, LMSW, LGPS, LGMFT)
- Must have master’s degree from an accredited University
- Must be able to meet and maintain the CCHS credentialing requirements
- Minimum of 2 years' experience providing therapy to adults, children, and families

Duties/Responsibilities:
- Addresses referrals from Choptank Health Medical and Dental providers, including referrals via warm hand-offs
- Participates in multidisciplinary meetings to assess needs and coordinate services
- Maintains weekly scheduled appointments in our electronic records system
- Provide therapeutic services to patients and their families in collaboration with multidisciplinary team members including external partners
- Completes required documentation and implementation in a timely manner. Documentation includes intake paperwork, biopsychosocial assessment, diagnostic screens, and assessments as necessary, treatment planning, discharge planning, satisfaction survey, and community referrals when needed.

- Maintain an active client caseload, based on the needs of the program and in accordance with COMAR regulations.
