  • Unit Management
    o Role models positive communication skills with nurses, C.N.A’s, interdisciplinary team,
    supervisory and management team, residents, and families.
    o Provides assistance and guidance to other members of interdisciplinary team in developing
    member care plans.
    o Listen to shift report and keep open lines of communication with supervisor, co-workers and
    on-coming shift.
    o Attend nurses and team meetings as scheduled and/or assigned.
    o Maintains professional relationships with physicians.
    o Responds to all emergency situations within the facility.
    o Encourages open discussions with residents and resident families.
    o Promotes rehabilitation nursing on the units.
    o Performs physical and psychological nursing assessments on residents when necessary.
    o Reports all unusual circumstances that may involve the safety and well-being of the residents
    and staff directly to the DON.
    o Demonstrates leadership ability and management skills through fair decision making,
    dependability and reasonable expectations.
    o Acts as House Supervisor at prescribed intervals.
    o May chart and/or co-sign shift documentation for nursing.
    · Case Management
    o Pre-admission Assessment
    § Performs pre-admission data collection and assessment.
    § Provides follow-up on member referral to the MDS Nurse, Social Worker and DON.
    § Performs an on-site or phone assessment.
    § Reviews and analyzes assessment and referral information in consultation with health
    team, family, member, discharge planners and insurance personnel.
    § Participates in the decision to accept member for admission based on treatment needs,
    cost-analysis and bed availability.
    § Provides timely feedback to payer and/or referral source.
    § Assesses family’s knowledge basis health status, expectations and potential discharge
    plans.
    § Creates with interdisciplinary team, a projected treatment and discharge plan.
    § Performs per diem, per pay cost-analysis for anticipated services necessary to achieve
    discharge.
    § Negotiates per diem as necessary, level of care and length of stay authorization with
    external payer.
    § Arranges for necessary equipment from assigned vendor, tracks equipment costs.
    § Keeps the MDS Nurse updated on residents conditions.
    § Collaborates with the clinical team in establishing a treatment plan.
    § Coordinates the treatment plan for short term clients which may include completions of
    the RAI assessment, care plan development, facilitating care conferences and
    summarizing clinical team interventions; provides appropriate oral or written
    documentation to external payer.
    § Coordinates the establishment of documentation guidelines for reimbursement,
    monitors documentation for accuracy/appropriateness, etc., in absence of the MDS
    Nurse.
    § Monitors resident progress and maintains high level of communication between payer,
    physician, family, member and clinical team.
    § Obtains as necessary, reauthorization for continued stay, secondary authorizations for
    education, equipment, consultations, etc. or reimbursement changes as resource
    utilization increases.
    § Facilitates and collaborates discharge teaching/planning with the clinical team,
    resident/family.

    o Discharge
    § Assures that all appropriate referrals are made, equipment delivered, and
    member/family training performed.
    § Collaborates with external payer and Social Services for all services to be provided upon
    discharge.
    § Collects all data necessary for outcome management.
    § Analyzes services delivered during the course of treatment and makes revision in
    collaboration with staff to improve process.
    § Performs post discharge profit/loss by member/diagnosis/length of stay/payer.
    § Determines cost of care by same categories.
    · Administrative Functions
    o Communication with business office regarding member coverage issues.
    o Provides training to the staff on in-house case management system, managed health care
    reimbursement models, Medicare/PPS along with performance expectations of external payers.
    o Participates in the facility continuous quality improvement program.
    o Provides timely feedback to facility and region regarding payer trends, alignment
    opportunities, competitive trends, quality perceptions and satisfaction.
    o Facilitates Medicare meetings and maintains records and data on clients in the absence of the
    MDS Nurse.
    o Serves as a resource to nursing personnel, other departments, families, residents, and other
    health care providers.
    o Assists with chart audits for regulatory compliance.
    o Attends educational meeting and seminars to promote personal and professional growth.
    o Keeps the Director of Nursing informed of relevant issues in areas of expertise.
    o Knowledgeable of emergency/safety procedures and policies and directs/assists staff and
    residents as the need arises.
    o Understands and adheres to State/HCFA/OSHA regulations.
    · Other Duties
    o Serve as directed on various quality assurance committees of the facility and provide written
    and/or verbal reports of the activities as required by the committee guidelines or directions.
    o Facilitates the weekly IDT meetings.
    o Attend departmental and facility staff meetings.
    o Supports Nursing Administration in their absence.
    o Participates in nursing on-call rotation per facility policy and will work weekends and holidays
    according to nursing schedule.
    · Performs other duties as assigned.
